When Going Home Isn’t an Option
Leaving the hospital should be the beginning of healing—not the end of care.
For patients who are medically ready to leave the hospital but have no safe home, no family or caregiver, or need continued assistance with recovery, navigating what comes next can be overwhelming. No one should be discharged to uncertainty when they still need support.
Our goal is to help connect individuals with the safe housing, supportive care, rehabilitation, medical follow-up, and community resources they need to continue healing and regain independence.
Because recovery doesn’t stop at the hospital doors—and everyone deserves a safe place to heal.

Housing Navigation
Our team provides personalized housing navigation and long-term housing support, helping individuals identify safe, stable, and sustainable housing options.
We work alongside each person to develop a plan for the future and connect them with the resources and assistance needed to achieve greater stability and independence.

We operate 2 facilities within San Diego County and also serve individuals from Los Angeles County, Orange County and surrounding communities.
Eligibility for Services
You may qualify for our program if you are experiencing homelessness, at risk of losing housing or living in an unsafe or unstable environment. We also serve individuals who have recently been discharged from a hospital, skilled nursing facility or psychiatric setting, as well as those with medical needs who require short-term support.
